Resource Coaching

This is where we come in. As resource coaches, we will ask for some basic information about your loved one. Based on the information you provide, we will recommend several professionals from various categories, including in-home care, adult day programs, caregiver support groups, geriatric care management (oversight), transportation services, respite care, eldercare attorney expertise, financial guidance, etc. We can help you understand each option and guide you toward those that best fit your situation.

We have excellent relationships with hundreds and hundreds of local area support services related to seniors. For example, if you have the need for a financial planner or mover/organizer, etc., we can connect you with several from our trusted partner network. And if we don’t have a resource for your needs, we can find one.

Whether you are close by or at a distance, we can provide you with all the necessary resources to assist your loved one in remaining safe and comfortable in their home. 

It’s true that a lot of information is available online. Most people get stuck not because they can’t find information, but because they’re not sure which information to trust or how to use it. Our resource coaching gives you vetted options, context, and a clear next-step strategy—so you can move forward faster and with less stress. 

Yes. The consultation is two hours. Based on all the information you provide, we will recommend various people and organizations to assist you and your loved one, usually in the home. The first 15 minutes are at no cost to you, so you are really paying for only 1 3/4 hours of the total 2-hour consultation. Following our two-hour consultation, we’ll give you a set of tailored options and resources, and from there, you’ll be able to continue on your own. 

Senior Living Placement Services

Senior referral agencies, also known as placement agencies or eldercare advisory services, help older adults and their families find the best retirement community, independent community, assisted living, adult family home, etc., for their needs. They provide customized recommendations, education, and support, resulting in better-informed, long-lasting decisions. Most agencies are paid by their partnering facilities when the older person moves in. This enables them to offer their services at no direct cost to the aging adult or their family members. 

We make a difficult job easier! We take the time to get to know you, your family, and your extended care team. We gain an understanding of the details of the needed care, finances, and desires, and work together with all parties involved until the best place is found. With over 20 years of experience in senior living and senior care, plus established relationships with hundreds of local senior care providers, our owner has access to information about care providers that can’t be found anywhere else. We ensure you are educated and know what you are signing up for. The end result is a new home that is long-lasting and just right. 

Short answer—no. The ability of an agency or organization to serve its clients well depends on its experience level, knowledge of the social, health, and financial needs of seniors, compassion, and connection to resources and technology. It is best to work with an organization like Aging Adult Transitions that conducts personal on-site visits of providers and works with their clients in person as well. In Westchester County alone, there are 41 adult care facilities (as of 2025) and 44 skilled nursing facilities, while in Fairfield and Rockland Counties, many more. The quality of a senior referral agency depends on the care and caliber of the individuals working there and their ability to keep track of a huge volume of information in a searchable, meaningful way. We are well-versed in all the senior living and skilled nursing homes in the area. We visit them regularly to obtain updates on programs, quality of care and medical access, health and safety, access to amenities and activities, and cost.

Our priority is helping you or your loved one find the ideal retirement community. We start by getting to know your loved one’s care needs, preferences, and budget, then identify communities that are the best fit. From there, we schedule tours, accompany you during visits, and act as your advocate—answering your questions, addressing concerns, and guiding you through every step of the decision process, ensuring all your questions are fully addressed. We can help with paperwork and assist in any necessary negotiations. And we will accompany you on the day of move-in for a brief period of time to support you. 

Our owner possesses in-depth knowledge and hands-on experience across all aspects of senior living, including independent living, assisted living, memory care, and continuum of care communities (CCRCs), as well as skilled nursing facilities and home care. As part of placement services, we thoroughly evaluate each community. We carefully review state inspection reports to inform you of any citations or concerns related to specific providers. These placement services are provided at no cost to you, as we receive compensation from the senior living communities.

For placement services, we receive compensation from the local retirement and assisted living community partners that pay us upon your move-in. That means you, our client, never receive a bill. You never have to pay us for senior living placement services. 

A phone call is usually the best way to start. You can call Monday through Friday, 9:00 AM to 5:00 PM, at (914450-9353 for a free 15-minute personalized consultation. During this phone meeting, we will conduct an in-depth discussion of your and your loved one’s needs, location preference, financial situation, and more. The next step is to meet in person or virtually so we can get to know your loved one personally and provide a tailored list of potential senior living communities and resources, such as independent living, assisted living, memory care, residential care homes, etc. Once you have selected a few potential communities, we will schedule and attend tours with you. We will guide you every step of the way until you choose the perfect fit for your loved one. And even after Mom or Dad has moved into a senior community, we will follow up to make sure your loved one is happy. 

Our owner, Brenda, has extensive experience working with older adults and their families, senior living communities, and skilled nursing facilities, with over 20 years of experience working with the senior population in both the senior living setting and the wellness arena. 

Brenda is also a Certified Dementia Practitioner, having completed specialized training and education in Alzheimer’s disease and dementia care from the National Council of Certified Dementia Practitioners. Through this training, she can help identify the memory care community that best fits your loved one’s needs, ensuring personalized care informed by specialized dementia knowledge and delivered in a supportive, dignified environment. 

This depends on many factors. We have facilitated emergency moves in a few days when safety has been a concern. Conversely, we have worked with families for many months before the aging adult is ready to make the move. On average, the process takes a few weeks to a few months to visit and find the right place and make all the move-in arrangements. Sometimes, scheduling a doctor’s appointment or providing all the necessary paperwork required by the Department of Health can delay a move and take extra time. If you are planning a move to a senior community soon and your loved one has not seen a doctor for a while, we suggest scheduling an appointment now. 

We make an effort to stay actively involved to help the transition go smoothly. We will be present for a short period on the day of move-in. Then, after the move, we check in with the provider and with you. If you experience any challenges during or after the move-in, we are always available to help and encourage you to contact us. 

As mentioned earlier, make an appointment with your loved one’s primary care doctor to sign the physician’s orders for your medications and supplements. Specific forms must be filled out by the physician in order to move to a senior living facility. These forms are required by the Department of Health. Also, you will need a nursing assessment and care plan from the community. Another hold-up can be the requirement for tuberculosis (TB) testing. The CDC recommends that people moving to a communal setting undergo TB testing within 30 days of the anticipated move.
